A few problems with your form.
1. Her species states she is an aquavita but her appearance and description say she is a Ventus Bird. If she is form ruling the Pluit pack she cannot have the appearance that she has. You can find species in the Guidebook. All species with the word Fonsaqua of just F. in their names may rule the Pluit pack.

2. The list thing aint working so can you take all her personality traits and copy-paste them so that they have spaces in between them.

3. She needs to have at least three past events plus a four month old pup left alone to fend for herself makes it very unlikely she would survive unless she finds a foster mum.
